Complete and Submit FAA form 8500-8 on the MedXPress website. When you submit the form the system will generate a confirmation number. Please print a copy of the filled-in form and that number and bring it with you to the exam. A Getting Started Guide is available here.

If you have an existing Special Issuance, bring (Before the due date listed in the letter!):
The FAA Authorization letter
A copy of all the Medical Test Results and other documents required by the letter
If you have an existing Chronic Medical Condition, bring a recent evaluation from your doctor
If you have an existing Statement of Demonstrated Ability (SODA), bring it
If you wear glasses, Bring them! If your vision has changed recently, have your eyes checked and a new set of prescription lenses made Before your flight physical.
If you recently started a new prescribed medication, check its FAA status here before your appointment. If the drug is disallowed by the FAA, consult with your primary physician to see if there is an acceptible alternative.
If you know your blood pressure or sugar has been slowly creeping up, get evaluated and treated before your physical! Most treaments for hypertension are acceptible to the FAA. Stay healthy to fly!
On the day of your appointment, Do Not take medicines or food which will spike your blood pressure or blood sugar. This includes nasal decongestants, stimulants, cigarettes or other tobacco products, or mass quantities of sugary foods and drinks.
